I would remove penis and work on arms and legs as they are not as cute as the rest of the picture.

Thanks for the feedback! That's interesting. I'm pretty happy with how the arms and legs turned out. I like the choppiness and detail of it as it makes it different from the original and more my style, but still arguably a correct cartoon representation of a naked toddler trapped in a scary basement.

What is the green thing behind them? And the sword is overanime styled and strange angle+ width.

Green smoke hehind is flare signal bombs;
I want to show the speed of the swinging sword so I bend it into large angle......

It's facing you, but the pose is kind of confusing indeed, but i didn't have time to fix it :D. I'll do it better next time. ;)
